Greetings!

This is to respectfully request a non-quota immigrant visa under Section 13, paragraph
A of the Philippine Immigration Act, as amended, in favor of ___.

I, ___, the applicant, and a citizen of Australia, and ____, the petitioner, and a
resident Filipino citizen, would like to apply for the amendment from provisionary to
permanent non-quota immigrant visa by marriage.

We were married in ___ on ___ and we are both residents of ____.

Enclosed here are the copies of the following documents to prove the above-cited
information:

1. Petitioner’s PSA-issued Birth Certificate;


2. The spouses’ PSA-issued Marriage Certificate;
3. Photocopies of the pertinent pages of the applicant’s passport:
a) bio-page;
b) page/s showing:
b1. his immigration admission and its extension/s;
4. Duly accomplished CGAF-001-Rev 1;
5. Valid National Bureau of Investigation (NBI) Clearance;
6. Bureau of Immigration (BI) Clearance Certificate;
7. Joint Affidavit of Continuous Cohabitation of applicant and the petitioning spouse.
