Dear Commissioner:

We, CAO FAQIANG and NAME OF EMPLOYEE, both Chinese citizen and both of legal age
after having duly sworn on oath in accordance with law hereby depose and say that:

As an applicant for the ISSUANCE of Pre-arranged Employment Visa (Commercial)


under Section 9(G) of C.A.613 (The Philippine Immigration Act of 1940, as amended), may we
respectfully request that this letter serves as guarantee for all the documents submitted in support of
the application for issuance of Non-immigrant visa of Mr. NAME OF EMPLOYEE.

In connection with the foregoing statement, all documents that we have submitted are
genuine and legally obtained from corresponding government agency/ies;

We do not have any intentions at all to defraud the government or other party/ies.

We are executing this sworn letter to attest the foregoing facts relative to the authenticity of
