AFFIDAVIT OF INTENT TO RETURN

I, PEDRO PENDUKO , Filipino, of legal age, married , and with residence and postal
address at Sambag San Cebu , Philippines , after having been duly sworn to in accordance with
law hereby deposes and states that:

1) I am a citizen and residence in the Philippines since birth,

2) I am currently studying in the college of law in San Carlos University in Cebu, Philippines
and aspired to become a member in the Philippine bar someday;

3) I come here in Netherland as tourist last month to have a short vacation in this country;

4) However my passport have been accidentally destroyed of mutilated yesterday ;

5) I intent to return to my home country considering that my purpose in coming here in


Netherland is only for a vacation for one month;

6) Hence, this affidavit is executed to attest to the truth of the above-narrated facts and to
further testify to the veracity of the foregoing statement.

AFFIANT FURTHER SAYETH NAUGHT.
