Dear Mr. Pring,

It has come to our attention that there have been instances of insubordination and dishonesty in your conduct,
which are serious violations of company policy and undermine the trust and integrity essential to our
workplace.

 February 5,2024 - Inform to getting an Xray


 February 7, 2024 - Half day due get the result of his Xray ( Still no result submitted)
 February 8-9 - Absent due to Fever
 February 12,2024 - Half day due to his driver’s license is missing
 February 13, 2024 - Absent need to go to LTO to report his license.

Insubordination, including refusal to follow instructions, disrespect, or defiance, is unacceptable behavior that
disrupts the functioning of our team. Additionally, dishonesty in any form erodes the trust and accountability
necessary for a positive work environment.

We request your immediate attention to this matter. Please provide a written explanation regarding the
incidents in question. We value your perspective and expect full transparency in addressing these concerns.
This explanation should be submitted within 3 days of receiving it.

It is imperative that we address these issues promptly to uphold the standards of conduct and professionalism
within our organization.

Should you have any questions or require clarification, please do not hesitate to reach out to the HR
department.

We trust that you will take this notice seriously and provide the necessary cooperation as we work to resolve
these issues.
