ANNUAL PHYSICAL EXAMINATION FOR TEACHING AND


NON-TEACHING PERSONNEL

In pursuant to DepEd Memorandum Order No. 22., s. 2015 re ANNUAL PHYSICAL EXAMINATION of
Teaching and Non-Teaching Personnel and Civil Service Commission Memorandum Circular No. 17, s.
1989.

The annual medical is conducted to ensure that ALL TEACHING and NON-TEACHING PERSONNEL are
MUST FIT to PERFORM their assigned duties and functions.

Physical Examinations are the following:

1. CHEST X-RAY
2. Complete Blood Test (CBC)
3. Fasting Blood Sugar (FBS)
4. Urinalysis

For pregnant, please attached your OB-GYNE medical certificate for documentation purpose.

Note: External medical laboratories are highly suggested such as ULTRA LAB (near Davao Del Sur
Provincial Hospital) and People’s Laboratory (2 nd floor Monarca Bldg)

Please maintain COVID-19 health protocol upon doing so.

Submission deadline is on March 5, 2021. Kindly comply
