Contents of entry:

- Cardinal Elements of a police record:


o Who:
 Name of the suspect/s and name of victim/s
 Name of the witness/ eye witnesses
 Identity of the officer assigned in the case
o What:
 Date and time when the offense occurred
o Why:
 Possible motive/s
o Where:
 Place where the offense occurred
o How:
 Nature or action of the action of the offense
o Disposition of the case:
 Significant circumstances that aggravate or mitigate the offense
 Current status of the case

- Incidents to be recorded:
o All violations of laws and ordinances
o All calls of incidents
o All legal papers handled
 Warrant, subpoena, etc.
o All fire alarms, reports and information
o Movement of prisoners
 Movement of travel
 Action taken
o Cases of missing and found persons, animals, and properties
o Vehicular and other types of accidents
 Hit and run
 Accidents where two or more motorcycles, cars, and trucks are involved
 Traffic violations

- Procedure in making entries


o All entries in the police blotter shall be handwritten
o Only facts are entered in the blotter
o No erasures shall be made on the entries
o A ballpen or pen with blue or black ink is used for making the entries
o Misinterpretations in the blotter or any attempt to suppress any information therein are
punishable criminally and administratively
o Entries must be legibly written in long hand and consecutively numbered
o Every page of the blotter shall be consecutively or chronologically filled-up
 Should follow the chronological series of events in regards to the case
 Should stick with the proper series of dates on what transpired to the status of
the crime
 No line of space shall be left blank in order to avoid tampering
 Any development of a case to be reflected in the blotter should be a new entry
at the time and day it was reported
 In every shift, the duty sergeant, under the supervision of the duty officer or
complaint desk officer, shall make the actual entries in the blotter and at the
end of his tour of duty, both the duty sergeant and duty complaint desk officer
shall sign the blotter

Banilad, Cebu City, reported to this Police
Office that on or about 6:00PM of February 13,
2023, he was awaken with a noise derived
from his motorcycle, Yamaha Ninja with the
plate number ILY-3000, on the front region of
his resident outside and when he checked by
his window, he saw the person Natalie Marie
Evangelista, similar range of age, also a
resident in their village, escaping along the
main road of the village starting from Jerome’s
house. It is noted that Jerome did not give any
form of consent/authority to having Natalie
the access to utilize his motorcycle even
without his awareness.
