SUBJECT: JOINT LETTER OF REQUEST OF APPLICATION FOR CONVERSION TO


NON-QUOTA IMMIGRANT BY MARRIAGE

Sir:

I, TREASURE A. O, the Filipina Spouse of SANG HYUK O, a Korean National, as


evidenced of my Filipino Citizenship is a copy of my Certificate of Live Birth issued by the
Office of the Civil Registrar of Baguio City under Registry No. 9004992 and authenticated by
the Philippine Statistics Authority (PSA) as attached for ready reference;

We were legally married on June 9, 2011 before Hon. Presiding Judge, Edilberto T.
Claraval of the Regional Trial Court, Branch 60, Baguio City;

Our date of marriage was duly registered in the Office of the Civil Registrar of Baguio
City under Registry No. 2011-00881, copy of our Certificate of Marriage issued and
authenticated by the National Statistics Office is attached for ready reference;

In view of our marriage, may we respectfully apply for the conversion to non-quota
immigrant by marriage stated in Section 13 (a) of Philippine Immigration Act of 1940;
