AFFIDAVIT OF LEGITIMATION
We, ROSALIE N. JORDAN, and EDILBERTO P. GULBEN, BOTH OF LEGAL
AGE, Filipino, Married, and a resident of Brgy Bitoon Muabog Tabogon, Cebu City,
Philippines, after being sworn in accordance with law, hereby depose and state that;

1. That we are the Biological Parents of ROBERT N. JORDAN, born on October 20,
1996, at Brgy Bitoon Muabog Tabogon, Cebu, with Certificate of Live Birth
Registry No. 96-678;

2. At the time of birth, we were living together as common-law husband and wife for more
than Five (5) Years; hence, the child was born out of wedlock;

3. We got married on MARCH 3,1998, at the Tabogon Cebu, Cebu City, which was
solemnized by then HON. Arlando C. Serafin , by Certificate of Marriage with
Registry No. 98-4056;

4. That at the time of the conception of the said child, we were unmarried without any legal
impediment to marry each other;

5. By virtue of our marriage, we would like our son to be legitimized and carry out our
respective surnames.

6. This is to the interest of our child to be conferred with a legitimated status.

7. That we are executing this affidavit to establish the foregoing facts and for the purpose of
conferring a legitimated status of our child ROBERT N. JORDAN.
