JOINT AFFIDAVIT OF LEGITIMATION


 
            We, Jose D. Moreno Sr. and Roselyn Calayag, both Filipinos, of legal ages, married
and presently residing at Carnaza, Daanbantayan, Cebu, respectively, having been duly sworn to
in accordance with law, hereby depose and state:
 
            1. That we have been legally married to each other on February 20, 2009 in a civil
wedding ceremony solemnized by Deodino U. Zuniga in Caloocan City which marriage is duly
registered as Registry no. 2009-1718. A copy of our marriage certificate is attached hereto and
made an integral part of this affidavit.
 
            2. That prior to our marriage and outside of wedlock, a child, named Jose Calayag
Jr. was conceived and born on January 12, 1998 at Bahay Pare Neycauayan in Bualacan. Her
Birth Certificate is registered as 247.

            3. That at the time of the conception of the said child and on the birth of the child,
we were not disqualified by any impediment to marry each other, and, therefore, by virtue of our
subsequent marriage, the said child is now legitimated by operation of law, particularly Article
177 of the Family Code.
 
4.       That we execute this affidavit to declare the truth of the foregoing facts.
 

 IN WITNESS WHEREOF, I hereby affix my signature this 9th day of April 2022 at
Daanbantayan City, Cebu, Philippines.
