AFFIDAVIT OF LEGITIMATION
We, _______________ and __________________, both Filipinos,
both of legal ages, married and residents of Purok 6,
_______________, Bohol, Philippines, after having been sworn to
in accordance with law, do hereby depose and say:

1. That we are the biological parents of _____________, who


was born on November 1, 2011 at _____________, Bohol, Philippines
as evidenced by the attached copy of the child’s Birth
Certificate, marked as Annex “A”;

2. That at the time of his birth, we the biological parents


were not yet married;

3. That at the time of the conception of our child, there


was no legal impediment for us to contract marriage;

4. That we got married on August 14, 2017 at Dauis, Bohol,


Philippines, as evidenced by the attached Marriage Certificate,
marked as Annex “B”;

5. That by virtue of our subsequent marriage, the child is


now legitimated by operation of law;

6. That we are executing this affidavit to attest to the


foregoing facts, for purposes of complying with the requirements
in applying for legitimation by subsequent marriage of our son
_______________ and for all intents and purposes.
