Problem Presented:
The Client was admitted in Madonna Hospital, Cagayan de Oro City with a diagnosis of
Acute Bronchitis, Acute Rhinosinusitis with Post Nasal Drip Hypertensive Urgency; HCVD and
Diabetes Mellitus Type 2-Insulin Requiring. Although the client is ready to be discharged after
completing her medications, she is unable to do so due to an outstanding hospital bill.
The client and her family lack the financial resources to cover this expense, which has
prompted the creation of this Social Case Study Report.
The client and her family are living in their simple yet durable house in Brgy. Pagayonan,
Madamba, Lanao del Sur.
The client’s husband currently lacks of source of income, relying entirely on the financial
support of the client’s relatives for their daily sustenance. All the acquired support of the client’s
relative is spend for her medication.
The financial burden has worsened significantly following their child’s hospitalization which
has led to substantial medical expenses that they are unable to afford. The unexpected nature of these
cost has placed additional strain on their already limited financial resources. As a result, for their
family, further complicating their already difficulty circumstances. Unfortunately, with no financial
means to cover these mounting expenses, the family is in a precious position that threatens their
overall stability and well-being.

This case involves a wife, along with her family, facing challenging circumstance who
urgently requires financial assistance from the government to settle a hospital bill. Given the severity
of her situation, the client and her family have made a prudent decision to seek support from
government agencies and institutions that are designed to assist individuals and families in crisis.
These organization often provide crucial aid to those in need, helping them navigate financial
difficulties and access necessary resources during difficult times.
Based on the assessment conducted through collateral interviews and the review of relevant
documents, such as the certificate of Barangay/Indigency and the Medical Abstract, it has been
determined that the client or Mrs. Mindalano, Mona Sumang and her family are in urgent need of
government assistance. The evaluation indicates that the family qualifies as indigent, making them
eligible to receive cash assistance for the payment of the hospital bill. Any amount provided would
significantly aid in addressing their financial burden and help cover the outstanding medical expenses.
