III. PRESENTED PROBLEM:

Mrs. Aisah D. Limbona’s daughter Noraya and Son Edris Dimapinggun was found to
have duplicate in other region.

IV. BACKGROUND INFORMATION:

A. The family

Saidali and his wife Aisah are blessed with nien (9) children, among these children
are Edris and Noraya who were found to have duplicate in other region. They are enrolled in
school except those who are working, Saidali is a farmer and earning 6,000 pesos every 3
months of harvesting, his wife is helping him by planting vegetables like root crops,
malunggay, kangkong and sitaw in their backyard garden.

B. Environment/ Community

The family is living in Barangay Kalanganan Lower, Pantar Lanao del Norte. They
are living in a peaceful community. Their barangay road is accessible because it consist of
two lane roads but it will take 15 to 20 minutes of travel from the national highway.
V. ASSESSMENT:

The household was found to have a duplicate in other area. As per validation by the
undersigned, Mrs. Aisah promptly said that she has no knowledge about this case, as she
knows that she has only one 4ps grants and her children is the recipient of her grants.

Based on his payment history the household received more than P5,000. They were
informed that they will be deleted in the program once the other account received more than
5,000 pesos. They are active in comply the conditionalities of the program, their monitored
children are enrolled and they were present in every Family Development Sessions. The
family is eligible and indigent.

VII. RECOMMENDATION:

With the assessment above, the undersigned recommends that both of her children
Edris Dimapinggun and Noraya Dimapinggun under household id number 103523013-8062-
00049 should be retain and that the other alleged duplicate from other region should be
delisted since they do not have knowledge abou it.
