1. I have read the PNP Memorandum Circular (MC) No. 2024-023 on PNP
Regulations on Bearing and/or Sporting of Tattoos and I Understand fully the
limitations/regulations imposed by the PNP on bearing and sporting of tattoos;

2. At present the following are the existing tattoos on my body:
