having been sworn to in accordance with the law hereby depose and say:
1. That I am a bonafide member of the Philippine National Police and presently
assigned with the 11th Regional Public Safety Battalion;
2. I have faithfully submitted my SALN as required under Article XI Section 17 of
the 1987 Constitution and Section 8 of Republic Act No. 6713, the Code of
Conduct and Ethical Standards for Public Officials and Employees.
3. That the submitted SALN for CY 2011-2015 which are hereto attached were
true copies of SALN previously submitted.
4. That I am executing this Affidavit freely and voluntarily to attest to the truth
of the foregoing and for all legal intents and purposes it may serve.
