Facts:
Personally appeared to this office the above name reportee and request to put into
official record the above mentioned missing person. Accordingly, her mother leave their
house at about 3:00pm of September 3, 2020 and up to this moment failed to return that
prompting reportee to proceed to this office to report the said incident. Any information
