1-2 Block 7 Tagaytay Farmhill Subd., Brgy. Ulat, Silang, Cavite, do hereby constitute
and appoint REX S. MACAPAGAL, of legal age, Filipino, and a resident of Phase 3 Lot 1-2
Block 7 Tagaytay Farmhill Subd., Brgy. Ulat, Silang, Cavite , to be my lawful attorney-in-
fact, for us and in my NAME, PLACE and STEAD, to DO and PERFORM the following acts and
things, to wit:

1. To be my authorized representative with full authority to retrieve my Identification Card


and;

2. To be my representative with full authority for any of the related processes with regards
to the renewal of my Professional Identification Card.

HEREBY GIVING AND GRANTING unto the above individual ATTORNEY-IN-FACT full
power and authority to do and perform such other acts and things whatsoever requisite or
necessary to be done in and about the premises as fully to all intents and purposes and we
might or could lawfully do if personally present. HEREBY RATIFYING AND CONFIRMING the
said ATTORNEY-FACT shall lawfully do or cause to be done under and by virtue of these
