Paragraph 3, s. 2021 of DepEd Order NO. 12, s. 2021 states that…


From March 1-12, 2021, schools shall conduct intervention and remediation activities based on
the unique needs of the learners as determined through the results of the different forms of
Rationale assessments administered for the past two quarters. These intervention and remediation activities
shall be indicated in the two week home learning plan to be prepared by the teachers.

The following are the objectives of this Assessment

a. To determine the reading readiness of Grade 1 learners;


b. To assess how effective the reading materials provided to parent are in making

c. To be able to determine further interventions that need to be undertaken
based on the results of this assessment

1. The teachers shall coordinate with the parents as to preferred modality of assessment
whether virtual or face to face.
2.If face to face, the teachers and parents shall agree on the venue whether it would be at
Strategy their residence or at a mutually agreed place within their locality, e.g. Barangay Hall, or other
spaces provided by the Brgy. Officials.
3.Coordination with the Brgy. Officials shall be made before the scheduled reading
assessment.

The Grade 1 Teachers will comply with health protocols. Wearing of face mask/shield shall
be strictly observed, as well as physical distancing, and regular sanitizing of hands with
