ACKNOWLEDGEMENT AND AUTHORIZATION

Non-Solicitation
I applied with NUSTAR Resort and Casino / Universal Hotels and Resorts, Inc. (the “Company”) of my own free will and volition. I was not solicited by the company
or any of its officers and team members to apply, nor was I induced to violate whatever contractual obligations I may have with my previous employer. I agree to
indemnify the Company for any and all damages incurred by the latter arising from, or as a consequence of, or by reason of my breach or violation of this
representation.

Confidentiality and Non-Disclosure:

I undertake that, at all times, I will hold in strictest confidence all Confidential Information to which I will be exposed, regardless of whether such information is
transmitted to me verbally or in writing. Any communication, information, documents, emails, or materials (regardless of form) are considered “Confidential
Information” if they are designated as such, or, even if not expressly designated as such, if their nature is such that it is evident that they contain sensitive,
proprietary, confidential or non-public information that should not be disclosed to a third party. I undertake that:

1. I shall not use any Confidential Information for my personal benefit or for any improper purpose;
2. I shall not sell, trade, publish or otherwise disclose any Confidential Information to anyone by any means such as photocopying, reproduction, electronic mail or
through social media (such as Facebook, Twitter, Blogs) without the prior written consent of the Company;
3. Regardless of the outcome or status of my application, I shall continue to honor my non-disclosure undertaking herein in perpetuity;
4. Under no circumstance shall I discuss or reveal the Confidential Information to my past or present employer, or to whichever entity I will next file my
applications for employment, whether local or foreign, without the Company’s prior written consent.

Conflict of Interest
I represent and warrant that:
1. I have no business, professional, personal or other interest that would conflict in any manner with the interests or business of the Company or with the
performance of my duties and obligations to the Company;
2. That if any such actual or potential conflict of interest arises, I shall immediately inform the Company in writing; and,
3. I shall immediately dissociate myself from or sever ties with such individual or entity if, in the reasonable judgement of the Company, such conflict of interest is
material or will serve as an obstacle to the performance of my duties and obligations to the latter.

Data Privacy
I hereby give my express consent to the collection, processing and use by the Company of my personal data, including any personal data that I have provided or
may subsequently provide in the course of my application or my employment (as the case may be), for all purposes relating to my application or my employment
or in connection with the Company’s policies, procedures, business processes, business objectives, or compliance with legal or regulatory obligations. I also give
my express consent to the transferring and sharing of my personal data among the relevant departments of the Company or among the Company’s affiliates.

I hereby authorize NUSTAR Resort and Casino / Universal Hotels and Resorts, Inc. to make reasonable inquiries, verifications, and background checks from my
schools, former associates, employers, customers, and other references or sources.

I further consent that if employment is later on established between me and the Company, my personal data may be disclosed to or shared with service providers
who may be engaged by the Company to assist with the Company’s activities, including external auditors, banks, medical practitioners, insurance companies,
payroll processor, advisers, consultants, and agents. All such service providers will be bound contractually to keep such information confidential.

Acknowledgement
I hereby acknowledge that I am being processed for a position in NUSTAR Resort and Casino / Universal Hotels and Resorts, Inc. I certify that the statements
made in this application form are correct and complete to the best of my knowledge. I acknowledge that any falsity, concealment of any material information, or
misrepresentation shall be considered sufficient cause for dismissal, in case employment is established between me and the Company.
