AFFIDAVIT OF EXPLANATION

WE, CRISTINA C. DELOS AMA, MONALIZA B. CASIMIRO, and ANGELO DL.


FRANCISCO, all of legal age, Filipino and a resident of Talavera, Nueva Ecija after having been
duly sworn in accordance with law, hereby declare and state that:

1. We were appointed as Chairman, Poll Clerk, and Third Member, respectively, of the
Board of Election Teller for the May 2018 Barangay and SK Elections, at Brgy.
Dimasalang Sur, Talavera, Nueva Ecija, specifically Cluster No. 132, Precint No.
168A and 168B;

2. That after the conduct of the counting of votes, we put the election materials and
papers inside the ballot box and safely locked it to be submitted to the Treasurer’s
Office of Talavera, Nueva Ecija;

3. However, when we inspect the other papers and documents to be submitted to the
COMELEC Office, Talavera, Nueva Ecija, we found out that the Sealed Envelope for
the Valid Ballots, was submitted to the COMELEC Office, where it should have been
inside the Ballot Box submitted to the Treasurer’s Office;

4. That the non inclusion of the Sealed Envelope for Valid Ballots inside the Ballot Box,
was unintentional on our part, and not for any other reason or purpose;

IN WITNESS WHEREOF, we have hereunto affixed our signature this 23 rd day of May
2018, hereat Talavera, Nueva Ecija.
