CHECKLIST OF REQUIREMENTS
ORE TRANSPORT PERMIT

Mandatory Requirements in the Acceptance of Application


Submit the following in two (2) sets per requirements.

1. Letter request to transport ore samples specifying the following:


a. Destination
b. Transporter
c. Buyer
d. Commodity
e. Tonnage and Volume
f. Value
g. Source

2. Submit/present to the MGB Office the following:


a. Permit number
b. Buying Agreement/Purchase Order
c. Payment of excise tax, etc. (paid for the applied commodity tonnage)

Payment Fees in the Application


1. Application Fee:
Non-Metallic Minerals PhP1,000.00
Metallic Mineral PhP2,000.00
2. Verification Fee of PhP2000.00/man/day but not less than PhP6,000.00
3. Transportation and Incidental Expenses for verification team
