3. When: (Time of Confinement) OOA 1:30 a.m. of September 11, 2020.

4. Where: (Place of Confinement) Room No. 319, Mindanao Medical Center,


Barangay City Heights, General Santos City.

5. Who: PS April Joy Yanan Jordan, 31 years old, single, PSFTP MANDILAAB
Class 2020-02 Police Student and a transient resident of Tieza Street, Barangay
Dadiangas North, General Santos City.

6. Why: Feeling dizzy and vomiting

7. How: On or about 12:45 a.m. of September 11, 2020, while at the billeting located
at Tieza Street, Barangay Dadiangas North, General Santos City, PS Norambai B
Panayaman, 24 years old, single personally informed the duty Field Training Officer;
PSMS Erwin P Palmera that PS April Joy Yanan Jordan was feeling dizzy and vomiting,
duty FTO immediately checked on the status of the latter and later asked for the
assistance of Police Station No.1 patrol officers to ferry her/PS Jordan to Mindanao
Medical Center this city for immediate medication. At around 1:30 a.m. of September 11,
2020, PS Jordan was confined at room no. 319 of the said hospital. Receiving physician;
Dr. Sumndan initial findings as HYPERTENSION, further update on the diagnosis of the
patient will be forwarded at your office, meanwhile relatives of the patient are being
informed on the status of the patient.
