AFFIDAVIT

I , SAIMA D. IBAY, of legal age, Filipino and a resident of Barangay Poblacion 1, Parang,
Maguindanao, after having been sworn to in accordance with the law hereby depose and state the
following:

1. That I am applying for a Barangay Official Eligibility from the Civil Service Commission
pursuant to the CSC Resolution No. 1200865.

2. That I have satisfactory meet the required number of years in service corresponding to the
number of years of the appointing Punong Barangay.

3. I am also aware of the Constitutional prohibition for any appointive barangay official to
be a relative by consanguinity and/or affinity within the fourth (4 th) civil degree to the
Punong Barangay;
4. I, therefore, declare that I am neither a relative by consanguinity nor affinity within the
fourth (4th) civil degree of Punong Barangay Raga Tomawis-Beda,Al-Hadja, and I am
qualified to avail and be granted with BOE;
5. I am executing this Affidavit to attest the truth of the foregoing and for whatever legal
purpose/s this may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 20th

day of February , 2018 at Barangay Poblacion Uno, Parang, Maguindanao.
