I, ROGER BALDON TOLEDA, of legal age, Filipino citizen, married and with
residence and postal address at Purok 3 Villahermosa, Daraga, Albay after having been
duly sworn in accordance with law do hereby depose and state:

1. I am the registered owner of a motorcycle with side car, more particularly


described under LTO Certificate of Registration No. 12281245-4 as follows:

2. I already detached the side car of the said motorcycle for personal reasons;

Land Transportation Office (LTO).
