P-22 Nursing PTO Policy (1)
UR/Jones expects employees to be at work as scheduled and to avoid unscheduled absences whenever
possible. It is the responsibility of each employee to follow his/her assigned schedule and arrive with
enough time to start working at the schedule time. Absence and tardiness without previous permission or
without notifying the Nursing Office or Administrative Coordinator may be subject to corrective
disciplinary action up to and including termination. All employees must coordinate time off with their
Department Director. Attendance (including unscheduled absences or tardiness) will be monitored on an
ongoing basis. The Department
Tardiness
Definition: Swiping in later than your scheduled shift.
Employee should be at their assigned area and ready to work at the start of their shift. In order to allow for
this, an employee can swipe in up to 7 minutes prior to their scheduled start time.
If an employee swipes in after their scheduled start time, they will not be allowed to extend their shift to
make up for lost time. An employee cannot clock in prior to seven minutes starting their regular shift.
PTO/Vacation time will be used to cover all occurrences of tardiness reaching and exceeding 15 minutes.
This is not to be interpreted as a grace period and will be considered a performance issue.
Employee is responsible for notifying supervisor in the event they will be tardy.
Employees must submit a completed Paid Time Off request to their department director/supervisor in a
reasonable amount of time, according to department guidelines, prior to anticipated time off.
All scheduled time off requires prior approval. The department managers must approve and the Paid Time
Off request to the employee within a reasonable time frame indicating approval or denial. Every effort shall
be made to grant time off that is requested in advance. However, staffing constraints and/or department
activity may result in denial of time off as requested or previously approved .2C, ICU, OB, ED one person
will be granted time off with each shift rotation (i.e. Day/ Night) unless Per Diem staff have agreed to
work the shifts. Time granted will go by date requested.
No employee may take paid time off in excess of the amount of benefit hours accrued as of the date/pay
period the time off is taken. Accrued benefit hours must be applied when taking off a day or more from
scheduled work.
