11. DESCRIPTION OF MODIFICATION:

The purpose of this modification No. P002 is to:

Update Attachment 1 - Schedule


1) Section 1.2 to extend the completion date of the agreement from August 15, 2020 to August 15, 2022; and add paragraph (d) to indicate
the award is incrementally-funded;
2) Section 1.3 to increase the total estimated amount by $11,999,538 from $4,999,858 to $16,999,396; increase the total obligated amount
by $5,999,746 from $4,999,858 to $10,999,604; and add paragraph (e) to indicate the award is incrementally-funded;
3) Section 1.4 to revise the award budget to reflect the funding increase for program expansion;
4) Section 1.5 to update the notification requirements under performance monitoring reports;
5) Section 1.10 to incorporate revised NICRA;
6) Section 1.12 to designate a new Agreement Officer;
7) Section 1.13 to update Special Provision paragraph (a) on the code of conduct; and add paragraph (g) on covered material;
8) Section 1.14 to update Mandatory Standard Provision Nos. 8 and 12;

Update Attachment 2 - Program Description to incorporate the revised Program Description;


Update Attachment 3 - Standard Provisions to provide the full texts of the updated Mandatory Standard Provision Nos. 8 and 12; and
Update Attachment 5 - Pharmaceuticals to incorporate the new pharmaceuticals list.

Accordingly, the changes are effected as follows (see continuation pages):

12. THIS MODIFICATION IS ENTERED INTO PURSUANT TO THE AUTHORITY OF THE FOREIGN ASSISTANCE ACT OF 1961, AS AMENDED. EXCEPT AS
SPECIFICALLY AMENDED HEREIN, ALL TERMS AND CONDITIONS OF THE AWARD REFERENCED ON BLOCK #3 ABOVE, AS IT MAY HAVE HERETOFORE BEEN
AMENDED, REMAIN UNCHANGED AND IN FULL FORCE AND EFFECT.
13. RECIPIENT IS IS NOT REQUIRED TO SIGN THIS DOCUMENT TO RECONFIRM ITS AGREEMENT WITH THE CHANGES EFFECTED HEREIN.
