AFFIDAVIT OF ACQUISITION OF FOREIGN CITIZENSHIP

I, _______________________________, single, ______________ Citizen, of


legal age and a resident of ______________________________, after being duly
sworn to in accordance with law, hereby depose and state:

1. That I was born on February 17, 1983 in Mandaluyong City, Metro Manila,
Philippines of Filipino parents ____________________ and
__________________. A copy of the Certificate of Live Birth is attached herewith
as integral part of this Affidavit and marked as Annex “A”;

2. That under the 1987 Philippine Constitution, citizens of the Philippines are
those persons whose either parents is a citizen of the Philippines and as
such I was born natural born Filipino Citizen;

3. That I and _____________________, a Swiss Citizen, have been legally married


to each other on June 3, 2008 at a wedding ceremony solemnized by Hon.
Jovito C. Abarquez at the Regional Trail Court, ____________ and the said
marriage was registered in the City/Local Civil Registrar of ________, Samar on
June 6, 2008 under Registry No. _____________. A copy of our marriage
certificate is attached hereto and made an integral part of this Affidavit and
marked as Annex “B”;

4. That thereafter, my husband and I lived at _______________ Oberrienden


Switzerland, the place where I am permanently residing and had been staying for
the last twelve (12) years;

5. That when I got married to my husband, I became Swiss Citizen by operation of


law of the Switzerland. A copy of the Certificate of Live Birth is attached herewith
as integral part of this Affidavit and marked as Annex “C”;

6. That I am executing this Affidavit to re-acquire and apply Filipino Citizenship to


affirm the truthfulness of the forgoing facts and for whatever legal purposes it
may legally serve.
