Dear Atty. Norman G. Tansingco,

May we respectfully request from your honorable office to grant MOHAMMED NAEEM’s
conversion to temporary resident visa (TRV) for one (1) year of an Indian National married to a Filipino
citizen under the memorandum order No. ADD-01-038. Mr. Mohammed Naeem, an Indian National, and
I, ROKARNA T. NAEEM, a Filipino citizen, have been happily married for twenty-six (26) years and
have been blessed with four (4) children, all of which are Filipino citizens. We both are currently residing
at LOT 1-E, Atilano Drive, Don Toribio St.,Tetuan, Zamboanga City, Zamboanga Del Sur, Philippines.
Working abroad for a long period of time, my husband and I have been eagerly wanting to settle down
here in the Philippines with our beloved children, and my family. Only with the help of you and your
honorable office, can we finally settle down here as a family.

We are enclosing here copies of the following documents to prove the above cited information:

1. Duly accomplished CGAF;


2. Photocopy of passport bio-page, visa implementation page and latest admission with valid
authorized stay;
3. Birth Certificate of the petitioning Filipino spouse;
4. Marriage Certificate/Marriage Contract;
5. Valid National Bureau of Investigation (NBI) Clearance;
6. BI Clearance Certificate.

Hoping that you'll give your earnest consideration and understanding to this matter! Thank you
