AFFIDAVIT OF LOSS

I, ALVARO E. RIVERA, of legal age, single, Filipino and a resident of Francia


West, Tubao, La Union, after having been duly sworn to in accordance with law, do
hereby depose and state the following to wit:

1. That I was issued a Student Driver’s License by the Land


Transportation Office (LTO-Agoo) which is to expire on July 2, 2012;

2. That recently, I was driving my tricycle along Marcos Highway at


Amallapay, Tubao, La Union where I dropped off a passenger;

3. That when I reached for my wallet to get fare change, I discovered that
my wallet containing my Driver’s License Card, among others, is
missing;

4. Diligent efforts were exerted to locate the same but failed, of which I
presume that the same is now lost and beyond recovery;

5. The said Identification Card was not confiscated, revoked or cancelled


by any law enforcing authority or the issuing government agencies for
violation of any laws, rules and regulations;

6. That I am executing this Affidavit to attest to the truth of all the


foregoing facts in support of my application for the issuance of a new
Professional Driver’s License Card by LTO-Agoo and for all other
legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 5th day of


September 2013 at Tubao, La Union, Philippines.
