AFFIDAVIT
(Re: NO BUSINESS OPERATIONS/EMPLOYEES)

I, ANTONIO A. DUMLAO JR., of legal age, married, Filipino Citizen,


and with residential/business address at 141 M. H. Del Pilar Street, Tugatog,
Malabon City, after having been sworn to in accordance with law hereby depose
and say;

1. That I am the Owner-Sole Proprietor of MYXP INTERNET CAFÉ


located at the same address as stated above;

2. That although said business enterprises was registered as an


employer with the SSS as early as December 22, 2004, it has no
business operations/income since its registration because I have
not completed the computer equipments and gadgets to be needed
including the installation of telephone lines in the area;

3. That it was only on April, 2006 when we finally started our


operations by entertaining customers, thus the need to hire an
employee in the person of HILDA V. RAC, with SSS No. 33-
4881819-7, who was still connected with us up to the present;

4. That it is for this reason that said employee was only reported as
employed with our enterprise from April, 2006 up to the present;

5. That I am executing this affidavit for whatever legal purposes this


may serve.

FURTHER, AFFIANT SAYETH NAUGHT.
