AFFIDAVIT OF LOSS
I, JHON ROLIE B. PESCADOR, of legal age, single, Filipino, and a resident of
915 Camia St. V and G subdivision Consolacion Cebu, Philippines, after having been
sworn to in accordance with law hereby depose and state that:

1. I am a licensed driver holding a Driver's License issued by the Land


Transportation Authority (LTO) - Driver's License Renewal Center in Danao
city District Office

2. In the Morning of 03 March 2023, I was going inside in the Philippine air
force Brigadier General Benito N Ebuen Air base and I submit my Driver's
License for identification to Military Police of the base to enroll in my school.
When I was going to go out and claiming my Driver's License the Military
police said it was gone

3. Efforts were exerted to locate said driver's license, but in spite of diligent
search, it could not be found and the same is now beyond recovery;

4. My Driver's License has not been confiscated by the LTO, Police or other


Traffic Enforcers for any traffic violation.

5. I am executing this Affidavit of Loss to attest to the truth of the foregoing


statement, and to request for a replacement of my Driver's License, and
for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have here unto set my hand this 3rd day of March
2023 at Consolacion, Metro Cebu, Philippines.
