WHY CONDUCT AN INCIDENT
INVESTIGATION?
An incident investigation is a tool to
find and solve problems in your
environmental management system.

When one of the elements is not


functioning properly, an incident can
occur.

We investigate incidents to:


○ discover what caused the incident
(what part of the system broke down
and why) and
○ eliminate the causes to ensure that
employees/environment are not
harmed during their work and that

WHEN TO CONDUCT AN INCIDENT
INVESTIGATION?
● All injuries (even the very minor ones)
● All accidents with a potential for injury
● Events involving unsafe conditions or property
damage
● All Near Misses where there was a potential for
serious injury

Report in writing within twenty-four (24) hours
from the occurrence of any of the following
environmental incident to the concerned EMB
Regional Office, as the case may be:
○ Breakdown of any pollution source and/or control facility or
monitoring instruments
○ Release (e.g. leaks, spills, etc.) of chemicals or hazardous wastes
(listed in this Order as well as the Priority List of Hazardous
Substances as published by the Agency for Toxic Substances and
Disease Registry of the Department of Health & Human Services
of the United States of America) to environmental media

Report in writing within twenty-four (24) hours
from securing of the results of laboratory analysis
or from online monitoring instruments any
exceedance to effluent and/or emission standards:

The report shall include:


○ Date/Time of the Incident
○ Cause(s) of the Incident
○ Description of the Incident detailing the estimated amount of chemicals
or hazardous wastes, emissions, and/or effluents released to the
receiving environment
○ Interim/contingency measures to mitigate any potential negative impacts

...when human activities may lead to threats of serious
and irreversible damage to the environment that is
scientifically plausible but uncertain, actions shall be taken
to avoid or diminish that threat.
