JOINT AFFIDAVIT OF LEGITIMATION

husband and wife, respectively, both of legal age, Filipinos, and all residents of
_______________________, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and state:

1. That we are the biological parents of _____________________________

2. That at the time of his/her birth, we, the biological parents were not yet
legally married, hence, his/her status in the original Certificate of Live Birth
was entered as ILLEGITIMATE;

3. That at the of the conception of our child, there were no legal impediments
or disqualifications for us to enter into a valid marriage as provided for by
law;
