AFFIDAVIT OF UNDERTAKING

I, JEROME M. SACRAMENTO, of legal age, single, Filipino, presently residing


at Brgy. Parado, Dingras, Ilocos Norte, after having been duly sworn to in accordance
with law, do hereby depose and state:

1. That I am a Fire Officer I (FO1) applicant of the Bureau of Fire Protection


Region 1;

2. That I am a graduate of Bachelor of Science in Criminology and holder of


Registered Criminologist License eligibility;

3. That I was born on April 20, 1996 as appearing in my Birth Certificate


issued by the Philippine Statistics Authority;

4. That I have not been dishonorably discharged from military employment


or on Absence Without Official Leave or Dropped from Rolls from the PNP
service or dismissed for cause from any civilian position in the Government;

5. That I have no pending criminal/civil/administrative case/s in any court,


including at the Office of the Ombudsman;

6. That I have not been convicted by final judgment of an offense or crime


involving moral turpitude;

7. That I further certify that the foregoing declarations are true and accurate to
the best of my knowledge and ability and I hereby authorized the BFP to
obtain and secure pertinent records that I will show my qualification standards,
collateral requirements and case records and other record related to these
declaration if any, in any agency or office.

FURTHER AFFIANT SAYETH NONE.
