Instructions & Additional Information

To accept funds from a third-party account (i.e. account that is not under client’s name), the Third-Party
person and our Client is required to complete and submit this authorization form, as well as provide following
documentation:
• Client’s verification documents, i.e. Proof of Identity and Proof of Residence
• Proof of Identity of the third party: This can be either a valid international passport or national ID
card or a driving license. The document must be clearly readable, and copies of front and back side
must be provided.
• In case this is a card deposit, then also copies of the credit/debit card used for the payment – Please
ensure to cover the sensitive information, leaving only the last 4 digits and cardholder name visible.
• In case this is a bank wire deposit, then also a copy of bank statement must be provided.

Finally, please be advised that, as per our terms and conditions, any future withdrawals must be made using
the same method used to fund your trading account and to the same remitter. It remains at Company's
discretion to accept/reject any future third party deposits. Please refer to the Company’s Terms & Conditions.
