Cromwell Z. Ong, general manager of Freedom Commercial Printers, signed an affidavit of undertaking for a government contract with the Department of Education Isabela City Schools Division. AFFIDAVIT OF UNDERTAKING

I, CROMWELL Z. ONG, of legal age, married, Filipino and a resident


of Zamboanga City, Philippines, after having been duly sworn to an oath in
accordance with law do hereby depose and say THAT:

1. I am the General Manager of FREEDOM COMMERCIAL


PRINTERS (hereinafter referred to as FREEDOM) with office
address at Gov. Ramos, Sta. Maria, Zamboanga City.

2. FREEDOM has been awarded a government contract by the


Department of Education Isabela City Schools Division (hereinafter
referred to as DepEd Isabela City), a government entity with office
address at Eastside, Isabela City, Basilan. Under the said contract,
FREEDOM bound itself to reproduce twenty-five million eight
hundred forty-two thousand, one hundred eighty-nine
(25,842,189) pages of modules for DepEd Isabela City who in
return will pay FREEDOM the amount of twelve million one
hundred forty-five thousand, eight hundred twenty-eight
pesos and eighty-two centavos (P12,145,828.82).

3. However, as of the moment, FREEDOM still has a remaining


balance of 4,844,000 pages of the modules still to be delivered.

4. To be entitled to the entire amount of the contract price,


FREEDOM promises that it will deliver the remaining 4,844,000
pages of printed modules to DepEd Isabela City on or before April
30,2021.

5. FREEDOM does hereby undertake to deliver the remaining


4,844,000 pages of printed modules to DepEd Isabela City on or
before April 30,2021.

6. I make these statements under oath and in good faith in order to


establish the fact that as the General Manager of FREEDOM, I
have the authority to execute this Affidavit of Undertaking for and
in behalf of FREEDOM; and that FREEDOM undertakes to deliver
the remaining 4,844,000 pages of printed modules to DepEd
Isabela City on or before April 30,2021.

7. I execute this Affidavit to attest to the truth of the foregoing facts


based on personal knowledge and authentic records and for
whatever legal purpose it may serve.
