NCP - Multiple Bruises

The patient presented with difficulty breathing, tachycardia, bruising, and open ankle fracture. Nursing diagnoses included ineffective breathing pattern and risk for impaired gas exchange. Interventions included monitoring vitals, encouraging deep breathing exercises, positioning, oxygen supplementation, and IV fluids as prescribed. Goals were for the patient to maintain adequate oxygenation, circulating blood volume, and demonstrate relaxation techniques. Evaluation showed maintained oxygenation and vitals after 15-30 minutes. Discharge planning included medication management, encouraging rest, and outpatient follow up.
