JOINT AFFIDAVIT

We, CHARITO I. MERINO and CONRADO E. LABARTINE, both of legal age, Filipinos,
and both residents of Villaba, Leyte, Philippines, after having been duly sworn to in accordance with law,
do hereby depose and say:

1. That we personally know VALENTIN DELIMA BAGUIO who was born on February 14,
1956 in Villaba, Leyte and our longtime neighbor and acquaintance in Villaba, Leyte;

2. That VALENTIN DELIMA BAGUIO is a bona fide Home Development Mutual Fund
(Pag-IBIG) and issued a corresponding Pag-IBIG ID number 82110006203P 002;

3. That appearing on his record in Pa-IBIG, it was entered herein as of his date of birth
February 14, 1960 and appearing to all of his pertinent documents, he was actually born on
February 14, 1956;

4. That the said discrepancy was purely due to inadvertent the same and for the reason that he
usually used February 14, 1956 and later on he found out that he was born on February 14,
1958 per Certificate of Live Birth;

5. We hereby declares under oath that by the name “VALENTIN DELIMA BAGUIO” born
on February 14, 1956 and the name VALENTIN DELIMA BAGUIO born on February
14, 1958, refer to one and the same person and that his true and correct name and date of birth
is “VALENTIN DELIMA BAGUIO” born on February 14, 1956;

6. We are not in way related to any of the foregoing person within the fourth civil degree either
by consanguinity or affinity, nor do we have pecuniary interest in this matter whatsoever;

7. We are executing this affidavit to attest the veracity of the foregoing facts and for whatever
other legal purpose this serves best.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this __ day of _________ at
