AFFIDAVIT TO USE THE USERNAME OF FATHER


I, JOHN LEO FERNANDEZ DICIANO, of legal age, single,
Filipino citizen and resident of #73 Maggay Street, Cataggaman
Nuevo, Tuguegarao City after having been duly sworn to in
accordance with law, hereby depose and say that:

1. I am the son of JANNIFER S. DICIANO and LEONILA M.


FERNANDEZ who was born on September 29, 1995 in
Tuguegarao City, Cagayan;

2. I was born out of wedlock and cohabitation of my parents;

3. I hereby declare and attest that I have chosen to use the


surname of my father DICIANO pursuant to RA 9255;

4. Said choice has been manifested by my school records and


other documents as JOHN LEO FERNANDEZ DICIANO;

5. I am executing this affidavit to attest to the truth and veracity


of the foregoing statements and for all legal intents and
purposes.

I, JENNIFER S. DICIANO, of legal age, Filipino citizen and


resident of #73 Maggay Street, Cataggaman Nuevo, Tuguegarao City
after having been duly sworn to in accordance with law, hereby
depose and state:

1. That I am the biological father of JOHN LEO F. DICIANO who


was born on September 29, 1995 in Tuguegarao City, Cagayan;

2. That at the time of the birth of the child, I was not legally
married to his biological mother;

3. That I hereby acknowledge paternity to the aforesaid child;

4. That I am giving my consent for the child to use my surname


“DICIANO” and that the same be reflected in his birth
certificate;

5. I am executing this affidavit to attest to the truth and veracity


of the foregoing statements and for all legal intents and
purposes.
