AFFIDAVIT OF SUPPORT AND GUARANTEE

I, RONALD CHUA WEE, Filipino, of legal age, married, and a resident of 28C Dr. Jr.
Estrada Street, Tetuan, Zamboanga City, after having been duly sworn in accordance with law,
hereby depose and state:

1. I am gainfully employed or engaged in business, holding a residence visa in China, with


the following pertinent contract, employment, and visa, with monthly salary of
______________;

2. I am sponsoring the visit of MAUREEN ANGELES ESPINOSA, of legal age,


Filipino citizen, with Philippine passport bearing No.
_____________________________________ hereinafter referred as the “subject”
who is the caregiver of my mother Alice Chua Wee;

3. I am guaranteed that I shall provide ALL financial support to pay for the food,
accommodations, and travel including airfare for return to the Philippines, medication
and hospitalization, and other expenses, debts, and obligations incurred including but
not limited to immigration fines and penalties of the “subject”;

4. I guarantee that the “subject” shall NOT in any way or manner what so ever be a public
burden or ward in the China or any country en route from or to the Philippines;

5. I guarantee that the “subject” shall NOT stay in the Republic China for a period longer
than the number of days allowed by China Immigration authorities;

6. I guarantee that the “subject” is visiting the China entirely for tourist and recreation
purposes and is NOT visiting the Republic of China for (1) employment (2) seek
employment (3) to transit via the China to another country where deployment of Filipino
nationals is restricted or where deployment requires clearance or endorsement of
relevant government departments and agencies in the Philippines.

7. That I am executing this affidavit of support and guarantee to attest to that truth of the
above-mentioned facts for the purpose of presentation to the truth to the Bureau of
Immigration in the Philippines.
