AFFIDAVIT OF WITNESS

I, Tom Holland, of legal age, single, and a resident of Barangay 1234,


Avengers Street, Manila City, after duly sworn to with accordance of law hereby
depose and state that:

1. That I am an employee of Chuva Chenes Logistics Services as


shipping and receiving personnel;

2. That on 26 May 2018, I was on my way to the company when Mr.


Manoban, husband of Mrs. Manoban, called me and handed me a
letter and asked me to give it to Ms. Matilda Matias De Leon;

3. That when I arrived at the company, I went to the respective office of


Ms. De Leon and gave her the letter from Mrs. Manoban;

4. That I executed this affidavit to attest the truthfulness of the foregoing


facts and to support the filing of criminal case against Matilda Matias
De Leon, for Qualified Theft.

day of June 2018 in Manila City, Philippines.
