SUBJECT: Request for Heavy Equipment Crane

With Reference to the above subject, I would like to inform your good office requesting
assistance for your heavy equipment crane and Mr. Emerson Tarra, for Installation, Commissioning of
Electro-Mechanical Pumping Facilities for our maintenance schedule to our well facilities located at
the following sites:

 Fiesta Communities Calulut – Hauling of 100 KVA Diesel Generator, then mounting to
genset room (September 04,2018)
 Fiesta Communities Castillejos – Pull Out/Change Over of Booster Pumps (Critical
Status) (September 04,2018)
 Fiesta Communities Mining – Change-over of Sunction Pipes, Pull out of Pump and
Motor (September 06,2018)
 Villa Sta.Barbara - Pull Out/Change Over of Pump and Motor (Critical Status) due to
vibration of Pump. (September 07,2018)
 Fiesta Communities Mariveles – Installation of Pump and Motor, Sunction Pipes and
Discharge Lines. (Sepetember 08,2018)

May we request your immediate response for your confirmation to the following schedule, we
understand that you also have a very tight schedule to your heavy equipment crane so I want to give
my sincerest apology.
The said activities will consist of pull out/install of Electrical Motors and Mechanical Pumps as
the primer load and the 100 KVA Electrical Generator as Prime mover together with the Column Pipes,
fittings and Discharge assemblies. It also contains wiring ins and terminations of feeder cables for
electrical lines.

for this request and wait for your immediate approval.
