Tribeca Medical Center Issues Notice Regarding a Potential Privacy Issue Involving its Patients

NEW YORK, Dec. 9, 2014 /PRNewswire/ -- We are providing this notice as part of Tribeca Medical Center's commitment to patient privacy. We take patient privacy very seriously, and it is important to us that you are made fully aware of a potential privacy issue.  We regret to inform you that on October 21, 2014, Tribeca Medical Center discovered a potential breach of protected health information. We are notifying individuals so you can take swift personal action, which along with our efforts, may help to reduce or eliminate potential future risk.

The incident involving protected health information was a theft of medical records stored in a custom built, double-locked storage facility in Jersey City, NJ.  On October 21, 2014 we learned that on or about that date, unknown individuals broke into the secured storage facility by cutting door latches and taking boxes containing medical records containing protected health information.

The facility primarily contained records of patients who had not been seen in our office for three consecutive years or thereabouts, between approximately 1982 and 2009, and were considered inactive.  The records of most patients who were seen in the office from January 1, 2010 through December 31, 2013 are stored at a different facility.  However, it is possible that a small number of active patients seen by the practice seen since 2010 may be affected by this incident.

Some of the medical records which were stolen contained protected health information including, but not necessarily limited to: patient names, addresses, telephone numbers, social security numbers, dates of birth, gender designation, insurance information, billing information, medical diagnoses, treatment records, laboratory and test results, and pharmacy/medication records.

This theft was immediately reported to the Jersey City Police Department. The Police Department responded to the scene, dusted for fingerprints and opened an investigation. Tribeca Medical Center will continue to work with the Police Department to assist them in any way possible with their investigation. To date, the information has not been recovered.  There are no longer any records stored at this facility.

If you have any questions, or need additional information, you may contact Dr. Nisar Quraishi at Tribeca Medical Center by toll-free telephone at 1-800-619-2738, through our website, www.tribecamedical.com, or via email at [email protected]

We are not aware of any indication that any unauthorized individual has used the information.  Nevertheless, it is recommended that you immediately take the following steps to protect yourself from potential harm resulting from this event:

  • Monitor financial and personal account statements, explanations of benefits, and credit bureau reports closely.
  • Register a fraud alert with, and order credit reports from, the three credit bureaus:
  • Every year, you are entitled to a free copy of your credit report from each of the three major credit bureaus.  For more information, visit www.annualcreditreport.com
  • New York residents may obtain more information about protecting yourself from identity theft from the New York Division of Consumer Protection at 1-800-697-1220 or www.dos.ny.gov/consumerprotection/
  • New Jersey residents may obtain more information about protecting yourself from identity theft from the New Jersey Office of Consumer Protection at 1-800-242-5846 or at www.njconsumeraffairs.gov/ocp

We sincerely apologize for the inconvenience and concern this incident has caused you. Your information privacy is very important to us and we will continue to do everything possible to correct the situation and ensure appropriate measures are being taken to protect your privacy in the future.

"A lot of times people will come to us and have a very diverse set of requirements or very customized need and we'll help them to implement it in a fashion that you can't just buy off of the shelf," explained Nick Rose, CTO of Enzu, in this SYS-CON.tv interview at 18th Cloud Expo, held June 7-9, 2016, at the Javits Center in New York City, NY.
For basic one-to-one voice or video calling solutions, WebRTC has proven to be a very powerful technology. Although WebRTC’s core functionality is to provide secure, real-time p2p media streaming, leveraging native platform features and server-side components brings up new communication capabilities for web and native mobile applications, allowing for advanced multi-user use cases such as video broadcasting, conferencing, and media recording.
Data is the fuel that drives the machine learning algorithmic engines and ultimately provides the business value. In his session at Cloud Expo, Ed Featherston, a director and senior enterprise architect at Collaborative Consulting, discussed the key considerations around quality, volume, timeliness, and pedigree that must be dealt with in order to properly fuel that engine.
The buzz continues for cloud, data analytics and the Internet of Things (IoT) and their collective impact across all industries. But a new conversation is emerging - how do companies use industry disruption and technology enablers to lead in markets undergoing change, uncertainty and ambiguity? Organizations of all sizes need to evolve and transform, often under massive pressure, as industry lines blur and merge and traditional business models are assaulted and turned upside down. Choosing the right cloud for your workloads is a balancing act that can cost your organization time, money and aggravation - unless you get it right the first time. Economics, speed, performance, accessibility, administrative needs and security all play a vital role in dictating your approach to the cloud. Without knowing the right questions to ask, you could wind up paying for capacity you'll never need or underestimating the resources required to run your applications.
As companies adopt the cloud-to-streamline workflow, deployment hasn’t been very seamless because of IT concerns surrounding security risks. The cloud offers many benefits, but protecting and securing information can be tricky across multiple cloud providers and remains IT’s overall responsibility. In his session at 19th Cloud Expo, Simon Bain, CEO of SearchYourCloud, addressed security compliance issues associated with cloud applications and how document-level encryption is critical for supple...
Whether your IoT service is connecting cars, homes, appliances, wearable, cameras or other devices, one question hangs in the balance – how do you actually make money from this service? The ability to turn your IoT service into profit requires the ability to create a monetization strategy that is flexible, scalable and working for you in real-time. It must be a transparent, smoothly implemented strategy that all stakeholders – from customers to the board – will be able to understand and comprehe...

Growth hacking is common for startups to make unheard-of progress in building their business. Career Hacks can help Geek Girls and those who support them (yes, that's you too, Dad!) to excel in this typically male-dominated world. Get ready to learn the facts: Is there a bias against women in the tech / developer communities? Why are women 50% of the workforce, but hold only 24% of the STEM or IT positions? In this strange new world where more and more power is drawn from business technology, companies are effectively straddling two paths on the road to innovation and transformation into digital enterprises. The first path is the heritage trail – with “legacy” technology forming the background. Here, extant technologies are transformed by core IT teams to provide more API-driven approaches. Legacy systems can restrict companies that are transitioning into digital enterprises. To truly become a lead...
Internet-of-Things discussions can end up either going down the consumer gadget rabbit hole or focused on the sort of data logging that industrial manufacturers have been doing forever. However, in fact, companies today are already using IoT data both to optimize their operational technology and to improve the experience of customer interactions in novel ways. In his session at @ThingsExpo, Gordon Haff, Red Hat Technology Evangelist, will share examples from a wide range of industries – includin...
While not quite mainstream yet, WebRTC is starting to gain ground with Carriers, Enterprises and Independent Software Vendors (ISV’s) alike. WebRTC makes it easy for developers to add audio and video communications into their applications by using Web browsers as their platform. But like any market, every customer engagement has unique requirements, as well as constraints. And of course, one size does not fit all. In her session at WebRTC Summit, Dr. Natasha Tamaskar, Vice President, Head of C...
In today's enterprise, digital transformation represents organizational change even more so than technology change, as customer preferences and behavior drive end-to-end transformation across lines of business as well as IT. To capitalize on the ubiquitous disruption driving this transformation, companies must be able to innovate at an increasingly rapid pace. Traditional approaches for driving innovation are now woefully inadequate for keeping up with the breadth of disruption and change facing...

The explosion of new web/cloud/IoT-based applications and the data they generate are transforming our world right before our eyes. In this rush to adopt these new technologies, organizations are often ignoring fundamental questions concerning who owns the data and failing to ask for permission to conduct invasive surveillance of their customers. Organizations that are not transparent about how their systems gather data telemetry without offering shared data ownership risk product rejection, regu...
When building large, cloud-based applications that operate at a high scale, it’s important to maintain a high availability and resilience to failures. In order to do that, you must be tolerant of failures, even in light of failures in other areas of your application. “Fly two mistakes high” is an old adage in the radio control airplane hobby. It means, fly high enough so that if you make a mistake, you can continue flying with room to still make mistakes. In his session at 18th Cloud Expo, Lee A...
